Lekesha Ransom, APRN, MSN, FNP-C has been in the healthcare industry for more than 15 years and she is
teaming up with local resources to assist in the preventative measures to increase health and wellness of people in
the community. Faith Health and Wellness Medical Center will offer prescreening for Diabetes, Hypertension and Cardiovascular Disease. These prescreening components will
include but are not limited to, individualized nutritional plan, weekly vital signs, blood test and monitoring Hemoglobin
A1c. Appointments are also available for primary care and other chronic care conditions.
We believe that these preventative screenings allow for early detection, which will lead to prompt treatment
thereby decreasing the likelihood of further organ damage and/or death.
